Paging Dr. LJ for new doctor

So, here's the situation. [info]quiet_elegance  has been going to the same PCP for about 8 years now. 
Preferred features in said doctor: 
near Lexington
willing to consider sending him to PT/pain specialist for his chronic knee pain*
not overburdened with Puritanical views on life.

Anyway- thanks in advance. There's not much I can do other than inquire amongst you, our community.

* The current  doctor has been disinclined to come up with a solution to the chronic back and knee pain. He will need knee surgery/replacement eventually, but in the meantime, he needs someone who is willing to say something other than "eat better, exercise, take ibuprofen." He's on his feet all day in high risk, hugely active manual labor as much as 12 hours a day. He's in too much pain to have a decent exercise option, and it's not like he's a desk jockey: he's moving around all day.
